Coast High School Independent Study Agreement

Proposed course of study:

Subsidiary contracts will be negotiated for each course that specify manner, time, frequency, date, and location, as well as the objectives, credit, time allotment, due date, materials needed for completion, criteria for evaluation, and other teacher, if assigned. Individual course objectives are consistent with the established HBUHSD Course of Study, activities, evaluation methods, and number of credits. These will be part of this master agreement. The student will complete the courses listed on this master agreement during the semester. Additional subsidiary contracts may be written as needed. Classes offered lead to a Coast High School diploma.

 

STUDENT: I agree to:

  • Be supervised by the credientialed teacher.
  • Meet with my teacher regulary according to the manner, time, frequency, date, and location specified in the subsidiary contract. The purpose of this meeting, called an appointment, is for me to report my progress by submitting my completed assignments by the due date to my teacher, and for my teacher to assess my learning through the evaluation of my completed assignments, provide instruction, and assign additional coursework. Failure to complete my assignments ithin 15 school days of the due date, unless an exception is made in accordance with district policy, or to behave inappropriately will result in an evaluation to determine if independent study is in my best interest and if I should remain. It may also result in one or more of the following: a phone call or letter of concern to me and my parent/guardian, a specially scheduled appointment or lab time, a conference with my teacher, a conference with the administrator, that my include my parent/guardian, placement on contract, revoction of any work permit issued until my school work is satisfactorily completed, termination of the aggreement and my return to a regular classroom program of instruction or other appropriate althernative.
  • Be responsible to arrange for transportation to school for my appointment. Lack of transportation is not an acceptable reason for failing to meet with my teacher.
  • Complete my assigned work and achieve at least the minimum performance requirements for each subsidiary contract. Credit which is based on mastery of learning can only be issued after I have successfully completed an assignment and it has been evaluated. I realize that a minimum of 16 hours of work is needed for each unit of academic credit, and that all work for each subsidiary contract is to be completed during the current semester.
  • Adhere to the District's non-discrimination/harassment and "zero tolerance" policy.

STUDENT: I understand that:

  • Independent study is a continuously voluntary educational alternative and that I have a continuing option of classroom instruction.
  • By entering Coast High School I have not vaived any rights as a student, and I am entitled to all HBUHSD services and resources.
  • I must follow all the discipline and dress codes and behavior guidelines of HBUHSD and Coast High School. Any violation of these guidelines or failure to meet district/school requirements could result in dismissal from Coast High School.
  • Independent study may be provided for a suspended student pending expulsion if the alternative of classroom instruction is offered.
  • Visitation on any other school campus requires prior permission from that school and a Coast High School administrator.
  • The regular high school program is 30 credits a semester for freshmen and sophomores and 25 credits a semester for juniors and seniors, with a maximum of 40 credits to be earned in any one semester and 10 credits during summer school. If I achieve only the minimum study requirements of 4 hours of productive work per school day, I will have attempted to complete at least the equivalent of one semester (5 credit) course per month. In order to make sufficient progress towards graduation I must exceed the minimum study requirements.
  • A maximum of 8 weeks for Fall and Spring semesters and 5 weeks during summer school is allowed between the assignment of a 5-credit class and the completion of the class by the student. I am entitled to a longer time to complete assignments if the superintendent, or designee, agrees with the special circumstances of this request.
  • Grades are issued based on work received and evaluated by the teacher. Each credit must have a passing score in order to receive a grade. Grades are assigned as follows: A 90-100%, B 80-89%, C 70-79%, D 60-69%, F below 60%. A grade of C or D is recorded for retakes. Final grades are given at the end of the semester and report cards are mailed home.
  • I must complete the assigned course(s), and I must check with my teacher before advancing to the next course.
  • I am responsible to return my current textbook and other class materials prior to starting a new class.

PARENT/GUARDIAN: I agree to the above conditions listed under "Student." I also understand that:

  • HBUHSD will provide the teacher services, textbooks, and other necessary items and resources as specified for each assignment.
  • Individual course objectives are equivalent in quality and quantity to the traditional high school program of the HBUHSD and are evaluated in the same manner.
  • I am liable for the cost of replacement and/or repair for willfully damaged or destroyed books and other school property that are checked out to or used by my son or daughter.
  • The teacher will meet with my child at a scheduled appointment to direct and measure my child's progress. The teacher, in consulatation with my son or daughter, will determine the time and location of the appointment. Independent Study is work-based. State funding is based upon the completion of student assignments within the prescribed time.
  • Te school administration evaluates my child's placement at Coast High School throughout the semester. It might be necessary for me to confer with the teacher and/or administrator when an issue with my son/daughter needs to be resolved.
  • I am expected to encourage my son or daughter to do more than the minimum study requirements, provide a suitable enviornment for learning, and to monior my son/daughter's daily progress. School attendance of my son or daughter is a parental reponsibility and failure to do so is an infraction of the Education Code.
  • I have rights and responsibilities as a parent/guardian under California Education Code in conjunction with the HBUHSD Parents' Rights.
  • I have the right to appeal any decision about my son's or daughter's placement, school program, or transfer according to HBUHSD's procedures.
  • The Education Code Section 48982 requires that the parent or guardian acknowledge that he/she has received the Parents' Rights notification.                                                                (revised 7/16/04)
